Gathering of health care data includes information regarding the physical, psychosocial, spiritual, developmental, and cognitive realms of the patient. Choose two of the realms of the patient and identify a minimum of three questions which you would ask of the patient. Upon identification, discuss the rationale with at least one example of what kind of information you expect to gather and why it would be important to the patient.
