Create an E-portfolio of important learning and resources each week through the course. You will share the E-portfolio with your professor at intervals and with your course colleagues in Week 7.  The final e-portfolio document containing all 7 parts, it will be submitted on week 7 for a grade.

Within Week 1, create a page entitled Who is in my community? and upload your work from E-portfolio 1.1. 2.
2.  Within Week 1, create another page entitled What is the health status of my community? and upload your work from E-portfolio 1.2.

1.    Within Week 3, create a page entitled Disease epidemiology exploration and upload your work from E-portfolio 2.1.

2.    Within Week 2, create a page entitled Disease trends in my community and upload your work from E-portfolio 2.2.

3.    Within Week 2, create a page entitled Home Environment Assessment and upload your work from E-portfolio 2.3.

Within Week 3, create a page entitled Community Health Improvement Plan and upload your work from E-portfolio 3.1.
Within Week 3, create a page entitled My Personal Preparedness Plan and upload your work from E-portfolio 3.2

1.  Within Week 5, create a page entitled Family Strengths & Characteristics and upload your work from E-portfolio 4.1.
2.  Within Week 5, create a page entitled Family Case Study and upload your work from E-portfolio 4.2
