Overview of Community Assessment and Community Health Nursing Intervention: Community assessment is a systemic way to determine the health status, resources or needs of a population. Community Health Nurses (CHN) assesses the community by using the nursing process:
Week 1: CH nurses gather subjective data (i.e. windshield survey and interviewing key informants). Based on this portion of the assignment you will analyze your findings and provide a summary of the key community health issues for your community.
Week 2: Next you will obtain objective data (Demographic, Education, and Economic date as well as Morbidly and Mortality rates, Health risk rates – obesity, alcohol use, seat belt use, etc), After collection and analysis of the data, Community Health Nursing needs are determined by comparing their community findings to the larger community (city/county, state and nation). Base on those comparisons, you will develop two priority community health nursing diagnoses.
Week 3: Lastly CH nurses develop interventions to address those priority health needs (CH Nursing Dx) and evaluate the effectiveness of the interventions implemented.
Overview: Windshield Survey of the Community and Interview Key Community Informants
The purpose of this assignment is for you to obtain a clear understanding of the physical environment, people and services within the community as well as to begin to understand the community’s perceptions of their needs. Before beginning this assignment, watch the following video. This windshield survey was conducted in London Ohio for a nursing course. This video is only one example of a windshield survey. You will survey similar items; however, your survey will not be exactly as the example.
In this assignment, you will conduct a windshield survey of a geographic area within or near the community where you live. Windshield surveys are effective in raising awareness of issues relating to public health in and around the community. Before you conduct the windshield survey, you may also wish to visit the site http://maps.google.com/. Input the zip code for your selected area. Once the map displays, view the map to determine if there are schools, parks, churches, bus stops, health care facilities, etc. in the vicinity. Note that Google maps includes both a traditional Map view and a photographic Satellite view of the area.
This assignment is due by Saturday, 11:59 PM of Week 1.
Directions
Locate a community focal/starting point (i.e. neighborhood, church, clinic, school, etc) in the city or county where you live. Once you select a focal or starting point, you will conduct a survey of the community surrounding the site. Specifically, you will survey a 10-block radius of the facility – 10 blocks north, 10 blocks south, 10 blocks east, and 10 blocks west. If something is NOT within the area of the community you are surveying (i.e. Hospital), note such and state how far away it is and how people will access it. If you do not see something within the community (i.e. homeless people) note no homeless persons were observed.
Additionally, you will survey two community members about their perceptions of the 2 health and 1 safety needs/issues and 2 strengths of their community.

Before you conduct the Windshield Survey, review the windshield form and the grading rubric so you will have a clear idea of what to look for.
Windshield Survey Considerations
If you do NOT use this template there will be a 30 point penalty and you will be required to resumit within 48 hours or a zero will be given. In order to do this assignment, you must put aside prior knowledge about your community and gather information as if you were observing places and things for the first time. You will be reporting on only those things you observe through the windshield. Use the information found in the INSTRUCTION document to complete this assignment to guide this observation experience.
Use a map of your county to decide what areas you will include in the windshield survey.
Use the attached assignment document to create a list of things to observe.
It is recommended that you request someone else to drive you. This will allow you to focus your attention on what you are observing.
Do not put yourself in danger. If at any point, you feel at risk or threatened, leave the area immediately. Your safety is of utmost importance.
While driving through the various regions, keep detailed notes about what you observe including the different kinds of buildings, businesses, service agencies, and recreational spaces.
Compare and contrast areas by looking at the road conditions, types of housing, people on the streets, rural or urban in nature, etc.
You will complete the tables included in the assignment document with the information that you locate. Your final report should SUMMARIZE what you find in area you survey.
When the assignment is completed then you will submit it to your faculty member for review using the Assignment tool.
