Each student will submit an individually written paper. Identify a public health/healthcare issue in a specific population. Your team leader has requested you to develop a plan to address this issue. Ensure that your needs assessment/background clearly delineates why this a pressing issue. Be specific in how you would go about developing and implementing your plan, the content of the plan, and the process/outcome measures you will use to evaluate its effectiveness. 1.Identify a public health/healthcare issue to be addressed in a specified population. (1 point)

2.Abstract: 250-350 words italicized, single spaced. (2 points).

3.Background/literature review: describe the epidemiological issues (incidence and prevalence, morbidity and mortality etc.). Comparison of the issue over time (at least 3 years) might be helpful to identify your choice. Consider your populations political, economic and social determinants of health that may be addressed in solving the issue. (6 points)
