The evidenced based tool that I have choose to discuss it the Illness Perception Questionnaire (IPQ). This tool is a set of questions answered by the patient that helps the clinician assess how the patients view on their illness affects their quality of life. The IPQ helps the healthcare clinician to monitor the patients ability to cope and offer appropriate interventions.  It has been used with patients who suffer many different severities and types of chronic illnesses from psoriasis to cancer. It includes questions about 5 different components of chronic illness; labels, timeline, cause, consequences and control (Arat et al., 2016). The patient answers eighty questions, on a scale from 0 to ten, on how much each component of the disease affects their life.  The IPQ can be administered to both adults and children as young as 8 years old. It has also been translated into as many as 26 languages (Broadbent et al., 2015).  Depending on the patient this can be a time-consuming assessment and therefore there are also revised and brief versions of the IPQ.  The brief version of the IPQ ends with an open-ended question about the perceived cause of disease.
